Safety Leadership and Psychological Safety: Why Your Most Experienced Worker Is Also Your Biggest Risk

Ask any safety director which worker keeps them up at night, and most will describe the new recruit: unfamiliar with the plant, unsure of the procedures, still learning where the hazards hide. The data used to back that instinct up.
It still tells part of the story. But it is no longer the whole story.
Look closer at serious injury and fatality data across construction, mining, energy, and process industries, and a harder pattern emerges. The workers most often involved in the incidents that actually kill people are not the nervous new hires. They are the veterans. The ones who have run the same procedure a thousand times, who know the plant better than the manual does, who stopped consciously thinking about the hazard around year three because their hands and feet learned to do the job without their brain in the room.
Safety researchers have a name for this. They call it complacency. I call it something more precise: a total loss of self-awareness at the exact moment awareness is the only thing standing between a routine task and a catastrophic one.
What Safety Leadership Research Says About Psychological Safety
I have spent my career studying self-leadership, not safety science. So before writing this, I went back to the research base that safety leaders actually work from, to see where the two fields meet. The overlap is bigger than I expected, and psychological safety sits right at the center of it.
Karl Weick and Kathleen Sutcliffe's work on high-reliability organizations is the foundation most serious safety programs are built on. Their five principles, preoccupation with failure, reluctance to simplify, sensitivity to operations, commitment to resilience, and deference to expertise, describe organizations that operate for decades in high-hazard environments without catastrophic failure. What strikes me most is that none of these are technical disciplines. They are all forms of collective mindfulness. They are what happens when individual people choose, moment to moment, not to let familiarity dull their attention, and when the people around them make it safe to say so out loud.
That last part is where the leadership research gets specific. Studies on safety-specific transformational leadership consistently find that when leaders build psychological safety, when they treat a reported near miss as information instead of a confession, workers report more of them, and organizations catch small problems before they become large ones. One study of clinical nurses found psychological safety was one of the strongest predictors of whether a near miss got reported at all, ahead of training and formal competency. Another, from the offshore energy sector, showed that "creeping change," the slow, almost invisible drift away from safe practice, is best interrupted by leaders who stay mindful and close to frontline reality rather than leaders who manage from a dashboard.
This is the part most safety programs get backwards. They build reporting systems and then wonder why the reports do not come in. The system was never the bottleneck. The bottleneck is whether a worker believes that speaking up about their own near miss, or someone else's, will be met with curiosity or with blame. That belief is not built by a policy document. It is built, incident by incident, by how a leader actually responds the first few times someone tells them the truth.
And then there is the number that should stop every executive in a safety-critical industry: depending on the study and the sector, somewhere between 70 and 90 percent of serious workplace accidents involve human error as a contributing factor. That statistic gets used constantly, usually to justify more rules, more signage, more training modules. But rules do not fail because people did not know them. They fail in the three seconds where a competent, well-trained, experienced person stopped paying attention to what they already knew, or noticed a risk and stayed quiet about it.
Neither of those is a knowledge problem. Both are self-leadership problems.
How Self-Leadership Builds a Culture of Psychological Safety
I built the three-competency model of self-leadership, self-awareness, self-regulation, and self-learning, long before I thought about applying it to safety. It was designed to help executives and knowledge workers manage attention, emotion, and decision-making in a world that constantly pulls them onto autopilot.
It turns out autopilot is exactly what kills people in high-hazard environments too, and it is also what keeps them silent.
Self-awareness is the capacity to notice, in real time, that you have stopped noticing. It is the difference between a technician who feels the familiar tug of "I've done this a thousand times" and recognizes it as a warning sign, versus one who simply acts on it. It is also what lets a worker register, honestly, that the shortcut they just watched a colleague take was worth flagging, instead of filing it away as none of their business.
Self-regulation is what lets a person catch that moment and consciously re-engage, rather than letting habit finish the task for them. It is the same capability that lets someone raise their hand in a briefing and say "I'm not comfortable with this" when every social instinct in the room is pulling them toward silence. Psychological safety is usually described as something a culture gives a worker permission to feel. In practice, it is also something an individual has to be internally equipped to use. A team can be told it is safe to speak up. Whether a person actually does, in the moment, under pressure, in front of a supervisor, is a self-regulation skill.
Self-learning is what turns every near miss, every close call, every uncomfortable audit finding into fuel for a better mental model next time, instead of something to explain away. It is the same engine behind the IGNITE Framework I use to help leaders convert setbacks into deliberate growth, and it is the throughline of my most recent book, POTENTIAL-IZE.
I talk about the Driver and Passenger distinction in most of my keynotes: are you driving your attention, or is your attention driving you? In a boardroom, that distinction affects decision quality. On a tank farm, a rig, or a factory floor, it affects whether someone goes home at the end of the shift. It is the same skill. The stakes are just brutally clearer.
High-reliability principles tell organizations what a resilient safety culture looks like from the outside. Self-leadership is the internal operating system that makes an individual capable of living those principles under pressure, boredom, fatigue, and the quiet overconfidence that twenty years of experience can produce. You cannot mandate mindfulness with a policy. You can build it, deliberately, as a trainable human capability. That is the work I do.
My Work Inside High-Safety Cultures: Vopak and Singapore Airlines
I want to be direct about where I am in this journey. Safety leadership is not a space I have built my career in. My clients have spanned aviation, tech, finance, and pharma, industries where the cost of losing focus is usually measured in dollars and quarters, not lives.
But I have worked closely with the leadership team at Vopak in Asia, one of the world's leading tank storage companies, where safety is not a compliance checkbox. It is the entire business model. I have also worked with Singapore Airlines, an organization whose global reputation rests on a safety culture built and defended over decades, at massive scale, in one of the least forgiving industries there is. Spending time inside both cultures, seeing how seriously experienced leaders take the discipline of staying alert and the discipline of keeping their teams willing to speak up, gave me a firsthand view of just how much of what determines safety outcomes sits inside a person's head long before it shows up on an incident report.
That experience, combined with what the research consistently confirms, has convinced me self-leadership deserves a seat at the safety leadership table. Not as a replacement for HRO principles, safety climate work, or behavior-based safety programs. As the missing layer underneath them, the one that determines whether a well-designed safety system, and a genuinely psychologically safe culture, actually gets lived by the humans inside it, shift after shift, year after year, long after the novelty of the job has worn off.
